What it measures

Rate of all recordable incidents per 200,000 hours

Lagging indicator

TRIR measures incidents after they have occurred. The operational failure and its associated costs (medical treatment, lost time, regulatory reporting) are already locked in before the rate is calculated.

Business impact

  • Increased 'chaos tax' from unplanned absence and restricted-duty workers, forcing reliance on expensive, less-trained agency labor to maintain shift coverage and prevent line stoppages.
  • A rising TRIR signals loss of operational control, triggering more frequent and intense regulatory inspections (e.g., OSHA, HSE) and customer audits, consuming management time and risking production holds.
  • Sustained high TRIR places the business on a formal supplier audit and performance remediation track with major retailers — the final warning before contract cancellation and delisting.

The real DAX measure HR-02 DAX measure from the Implementation Pack

This is the primary ratio the pack's other measures build on — trend, rank, and period-comparison variants, all on Fact_HSE_Incident_Event.

Data you need
  • Fact_HSE_Incident_Event — 7 mandatory columns — plant key, equipment key, ...
  • Dim_Employee — key + name
  • Dim_Equipment — key + name
  • Dim_Plant — key + name
  • Dim_Site — key + name
  • Dim_Date — Auto-generated calendar table — no customer data required

Full field-by-field data contract, in 4 SQL dialects (SQL Server, Snowflake, Databricks, BigQuery), ships in the pack.

Editable thresholds
Target1.5 — performance considered compliant at or below this line
Warning3.25 — amber alert — triggers a review

Both live in one Excel tab — change them, refresh the report, done. No DAX editing needed.
